Many of the Catholic Long Term Care Network members were founded as traditional “nursing homes,” providing care over long periods. In order to meet the identified community needs, and reflect the development of contemporary health-related practices, we now provide a broad range of services, not all of which are “long term.”


PRIMARY SERVICES
(Provided directly by CLTCN members)

Skilled Care
A level of care that must be given or managed by licensed health care professionals and is under the general direction of a physician

Intermediate Care
Health-related care provided under a “plan of care” for those not requiring “skilled care”

Personal Care
24-hour supervision in a residential setting

Memory Deficit Care
Care to meet the special needs of those who are cognitively impaired (such as people with Alzheimer’s disease)

Short Term Care
A limited period of care until a recovery progresses far enough to enable independent living

Assisted Living
A high level of independence, for which limited assistance is needed with the tasks of daily living

Pastoral Care
The opportunity for worship, prayer, emotional support, spiritual growth, and faith development, regardless of religious affiliation

Therapy
Physical, speech, and occupational therapies intended to rehabilitate and restore independence

(Provided by CLTCN facilities through agreements with outside organizations)

Family-support activities
IV therapy and oxygen therapy
Medical and psychiatric services
Pharmacy services

Hospice care
